The following resources are for ministry leaders or persons responsible for organizing an event at Calvary Chapel Oceanside. In order to insure your ministry event, please be sure to fill out the proper forms and submit them immediately. You can return the completed forms to the church office Monday through Friday, from 9:00 am - 5:00 pm, fax to (760) 754-4488, or fill them out and email them here.
